Amy McMillan Therapy, PLLC is an out of network provider for insurance and can provide you with a Superbill monthly statement of services. It is your responsibility to submit this form to your insurance company for reimbursement and to verify your coverage for therapy services before beginning psychotherapy.
Session Fees

Please remember to cancel or reschedule 24 hours in advance. You will be responsible for the entire fee if cancellation is less than 24 hours.

Individual Therapy
Personalized sessions focusing on your individual experiences, insight, and coping tools. A space to explore your emotions, patterns, and relationships in a supportive and collaborative way. We focus on how you relate to yourself and others, using the therapy relationship as a place for awareness, growth, and genuine connection.

Parenting Sessions
A supportive space for parents seeking guidance, co-parenting support, or help responding to a child’s behavioral or emotional needs. These sessions provide education, structure, and space to reflect and realign on parenting. The focus is on connection, confidence, and clarity as you navigate challenges, transitions, or co-parenting, while exploring ways to stay grounded as you supporting your child’s growth.

Family Therapy
For families navigating transitions, communication challenges, or conflict. Sessions often include multiple family members and focus on patterns and relationships within the family system. Family sessions help improve communication, repair disconnection, and strengthen relationships. Together, we explore patterns and build new ways of understanding and supporting one another within the family system.​

Good Faith Estimate & Dispute Process Disclaimer
Transparency in fees and charges is an important part of the therapeutic relationship. In accordance with the No Surprise Act, Amy McMillan Therapy, PLLC provides Good Faith Estimates to all clients. The Good Faith Estimate shows the cost of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that might arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill. Ultimately, Amy McMillan Therapy prioritizes working with clients and/ or their families to create a treatment plan that meets the client's emotional needs with clear communication about treatment costs. Frequency and duration of sessions are an ongoing and open conversation.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. You may contact the health care provider or facility to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.

You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date of the original bill.
​
There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on the Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
